Many fatal accidents have occurred when machines started to run when it was supposed to be completely off to prevent repairs or maintenance. These accidents are usually caused by the energy being not released correctly. Lock-out tag-out (LOTO) systems help prevent these incidents by ensuring that machines remain completely off until an authorized person releases them.
The LOTO system is made up of a number of locks, tags and other devices used to isolate, secure, or block a machine from an electrical source. All "affected" and "authorized" personnel must be trained in the use of these devices. These devices include, but are not restricted to chains, locks, and padlocks. Each device is identified and labeled, including the date and the time of application and the name of the person who is authorized to use it and the reason for the lockout or tag-out (repair maintenance, repair, etc.). It must be stated clearly that these devices can't be removed until a thorough LOTO procedure is followed.
